Since brain tumors rarely have stages but do have grades I assume that you mean a GBM grade 4. Based on that and your description I unfortunately have to tell you that he does have a very bad prognosis
Yes CyberKnife or GammaKnife treatment CAN be done without biopsy, though I myself would greatly prefer to have it done AFTER a biopsy so that I would know with some certainty what it is that I'm treating!
No, I do not think any symptom giving brain tumor can remain hidden from MRI scans. However without the possibility of doing examinations of my own I can hardly contribute to solving this true mystery
