Questions related to childrens birthday parties are my speciality - but I'm happy to do my best with any question - I'm in the UK, so if your question is specific to another country I'm unlikely to be able to help
I've organised parties on a professional basis since 1978!

The important thing with invitations is to ensure that the children enjoy the LOOK of the invitation and the parents get the INFORMATION they need in a clear manner. Tell the parents what time they have
It sounds like you already have a pretty fair plan – the important thing about any party is that people enjoy themselves, so doing things they are comfortable with is fine – if gambling is for them, then
My birthdays tomorrow, but that aside! MOST important thing for a birthday is to have fun, so do what YOU like with the PEOPLE you like - which can cost next to nothing - my best ever party was a meal
I always think it's important to be clear, so something along the lines of "You buy the meal, I provide the cake!" will be fine - I would set expectations such that they expect to pay for pretty much everything
I'm in the UK, but don't know where you are - which might be relevent to suggestions. Also don't know what you like to do, which is relevent to the best type of party to have. Also don't know budget
