I can help. I have installed wood, laminates, tile, marble, ceramics, vinyl, and painted applications. I have done some stone work, but that is seldom used anymore.

Sanded grout is for 1/8" or larger joints. You might do better with unsanded grout. It has the paste consistancy to bond in a thin layer. You are also advised to scratch out as much loose as you can
This is information of a delicate type. I imagine you have air/moisture intrusion from an outside source. A window leaking, a vent that is moldy or other such unseen situation if your moisture per room
Warping is not always a moisture issue. Sometimes it is expansion. Edges can get compressed an tweek. Heat can also affect an area. When I install laminates I make it understood, I glue All seams/joints
If you want a quality floor....stay away from Laminates. If you want the look of wood...go with engineered flooring. It is alittle more money...and alot more worth it. I have not seen ONE laminate
