From your suggestions, I am hoping we don't have disease, but we'll get it tested and go from there. And having properly prepared (amended as you stated) the soil would have been beneficial before planting overall. :) I am confident that they were hardened off as you noted since they were purchased from an outside nursery and a garden center... which also covers your last thought of buying from a different nursery.
